## Ways to pay less

- **One month free with yearly billing** (Annual discount): TheraNest's pricing page gives one month free on annual plans: Essentials is $319 a year instead of $348, Advanced $649 instead of $708 and Premier $979 instead of $1,068. Source: https://ensorahealth.com/pricing/theranest-mental-health/

## Key features

- **Scheduling and client portal:** Calendar management with personal calendar sync and staff availability, and a portal where clients see documents and appointments and pay online. Online booking starts on Advanced.
- **Notes and case management:** Custom client, case and progress notes, built-in clinical case management with ICD-10 tools, and Wiley treatment planning, which is included on Premier.
- **Insurance claims and eligibility:** Primary and secondary claims sent singly or in batches through clearinghouse partners, real-time eligibility checks, and billing automation on Premier.
- **Telehealth:** HIPAA-compliant video for individual and group sessions, unlimited on Advanced and Premier and a $12-a-month add-on per therapist on Essentials.
- **Ensai AI:** Ensai polishes progress notes on every plan. The Ensai Session Assistant, $35 a month per provider, scribes telehealth and office sessions into notes and writes case summaries.
- **Credentialing:** A specialist team prepares and follows up payer enrollments with no commission: one or two payers per therapist by plan, and one more on annual billing.

## Good for

- Insurance-based therapists who want claims bundled into the plan
- Practices that need help getting in-network with payers
- Group practices with schedulers and billers, who are free

## Worth knowing

- Advanced includes 50 claims, eligibility checks and reminders a month per practice; then claims cost $0.23, checks $0.25 and reminders $0.10 each.
- Add-ons: Ensai Session Assistant $35 a month per provider, Wiley Treatment Planner $25 a month below Premier, eRx $45 a month per therapist plus a $135 setup fee.
- Each therapist pays an AMA CPT license fee of $19.50 a year, billed in December; card payments cost 2.99% + $0.30.
- Premier's unlimited claims and reminders fall under a fair usage policy, and a downgrade takes effect only when the current contract ends.

## ToolGPA grades

**GPA: 2.2 out of 4**, graded Oct 24, 2026. TheraNest, now sold by Ensora Health, covers scheduling, notes, claims, eligibility checks and telehealth, and its team handles payer credentialing with no commission. Essentials is $27 a therapist on yearly billing, but online booking, messaging and bundled claims start on Advanced at $54, and AI scribing, eRx and a CPT fee cost extra.

- **Value for money: C.** Essentials is $27 a therapist a month on yearly billing, $29 monthly, between this category's cheaper quarter ($25) and middle ($39). Online booking and 50 bundled claims need Advanced at $54, and telehealth is $12 extra on Essentials.
- **Free plan and trial: C.** A 21-day trial of Premier with no card. There's no free plan.
- **Features: B.** Scheduling, a client portal, custom notes, case management with ICD-10 tools, primary and secondary claims, eligibility checks, telehealth, Ensai note polishing and a credentialing team. The job is covered, spread across three plans.
- **Ease of use: C.** No App Store rating on record. Intake forms, note templates, payer enrollment and claims all take setting up, though the credentialing team prepares the enrollments.
- **Platforms and integrations: C.** Web only, with no mobile apps named, and Wiley Treatment Planners is the one integration listed. No API on record.

- It stands best on features (B, above 1 of 12) and worst on ease of use (C, above 0).
- Its paid plans start at $27 a month, against a middle price of $39 for the category: 3 of the 10 others that charge start lower.
- On features, ease of use and platforms it scores 2.3 out of 4. 3 tools score higher and cost less.

## Keep it, or make your own?

**Keep it.** To replace TheraNest you would be making insurance claims sent to payers through a clearinghouse. That is not a weekend's work, with an AI app builder or without one. That makes this kind of software worth paying for, not this one in particular: 5 of the 12 other therapy and clinic software grade higher. It costs $324 a year.

- You could make: Clinical notes and appointment booking: an app builder makes these well.
- You could not: Insurance claims sent to payers through a clearinghouse.
- You could not: Video sessions that meet health privacy rules.
- You could not: Speech turned into text you can trust.
- You could not: AI features, unless you pay a model provider for each request yourself.
- You could not: What a company asks for before it lets staff use something: HIPAA.
- You could not: The depth: we grade its features B, which means it covers the whole job.
