What it is, who it suits, its grades and the ten things to know first

2.2

GPA, above 2 of the 12 other therapy and clinic software we grade

TheraNest is a mental health EHR and practice management system for solo and group therapy practices, now sold by Ensora Health alongside its Fusion product for rehab therapy. It covers scheduling, a client portal, custom intake forms and progress notes, insurance claims and eligibility checks, telehealth and payments, with Ensai AI tools for notes. Plans are priced per therapist.

To get you started, here are 9 things to know about TheraNest, taken from its own pages and our grades.

Intake forms, note templates, payer enrollment and claims all take setting up, though the credentialing team prepares the enrollments.

  • Watch out: Advanced covers 50 claims, checks and reminders a month, then 23¢ a claim, 25¢ a check and 10¢ a reminder
  • Pricey if you add the Session Assistant at $35 a month, eRx at $45 plus $135 setup, or Wiley below Premier at $25
  • Watch out: each therapist pays a $19.50 yearly CPT fee, cards cost 2.99% + 30¢, and Premier's unlimited has a fair usage policy
  • Skip it if you might downgrade soon, as the change only takes effect when your contract ends

It stands best on features (B, above 1 of 12) and worst on ease of use (C, above 0).

Keep it, or make your own?

Whether this is something you could build for yourself with an AI app builder, and what you would give up.

Keep it

To replace TheraNest you would be making insurance claims sent to payers through a clearinghouse. That is not a weekend's work, with an AI app builder or without one.

That makes this kind of software worth paying for, not this one in particular: 5 of the 12 other therapy and clinic software grade higher. It costs $324 a year.

What you could make

  • Clinical notes and appointment booking: an app builder makes these well.

What you could not

  • Insurance claims sent to payers through a clearinghouse.
  • Video sessions that meet health privacy rules.
  • Speech turned into text you can trust.
  • AI features, unless you pay a model provider for each request yourself.
  • What a company asks for before it lets staff use something: HIPAA.
  • The depth: we grade its features B, which means it covers the whole job.

What this rests on. TheraNest’s listing: what it does, where it runs, what it plugs into and what it costs. It is a judgement about the kind of software this is. We have not built a replacement for it, and making your own means looking after it too. How we decide, and every tool’s verdict.
