HeyForm is an open-source form builder that presents itself as an alternative to Typeform. It creates conversational forms for surveys, questionnaires, quizzes and polls, with conditional logic, multiple endings, payments and an AI form generator. It runs as a hosted service with a free plan and three paid plans, or it can be self-hosted from the source code, which is published under the AGPL-3.0 license.

HeyForm makes conversational, Typeform-style forms, and it's open source, so you can host it yourself. Basic is $15 a month billed yearly for unlimited forms and 5,000 responses. The pricing page doesn't say what the free plan includes, HeyForm takes 3% of payments on Basic and 1% on Premium, and team members, a custom domain and removing the branding all start at Premium.
