AStarter is $15 a month paid yearly ($180) for 2,000 responses, and every plan has unlimited seats. No per-person fee is rare in forms. Removing all branding needs Pro at $40, though, and your own domain needs Business at $75.
AIt's free with a personal Google Account. The business version isn't sold on its own: it comes inside Google Workspace, from $7 per user a month on a 1-year commitment, with Gmail, Drive and Meet in the same price.
